Please do not use this form if you have an urgent medical problem or you need to reschedule an existing appointment. Instead, contact our office at (860) 225-1227. After you submit your request, our appointment scheduler will respond within 48 hours. Please note that your requested date/time may not be available but we will do our best to find the first available appointment that fits your schedule. Please be ready to give the appointment scheduler your insurance information when they contact you.

Patient Registration Forms
Central CT Neurosurgery & Spine in New Britain, Connecticut, welcomes you to our practice. Once you have made your initial appointment, you may download and print the forms, fill them out, and bring them with you to your first visit. If you are unable to print the forms, let us know and we can mail the forms to you if there is enough time to do so before your appointment. Completing the forms prior to your visit will help speed up the new patient registration process.
Also, be sure to bring any diagnostic films or other test results that may have been ordered or performed by another physician.
